Retention

A relationship built to last.

Patients stay when care is easy to reach, someone answers the phone, and the team keeps reaching out between visits.

Our internal data shows documented patient attrition below 1%.

Based on internal clinic records.

A strong primary-care relationship gives members another reason to value and use their health coverage.

Access

Access should
feel simple.

  • Same-day appointments

Generally available for new members introduced by partners and for established patients when clinically appropriate.

  • Walk-ins welcome

Appointments are recommended for the fastest experience.

  • Real people answer the phone

During business hours, patients reach clinic staff — not a phone maze.

  • Fully bilingual team

English + Spanish across all patient-facing staff.

Two very different phone calls.

Traditional healthcare

  1. Call
  2. Voicemail
  3. Wait
  4. Transfer
  5. Callback
  6. Appointment days later

Chavez & Mederos

  2. Real staff
  3. Problem identified
  4. Routed internally
  5. Care coordinated

Navigation

One call.
One team.

Patients should not have to navigate healthcare alone.

One call reaches staff who identify the issue and route it internally — the patient never has to know which department handles it.

    Remote Patient Monitoring

When something changes — medically or administratively — the patient already knows who to call.

The goal is simple:

Keep the patient connected before a small problem becomes a bigger one.

  • Care gaps
  • Post-hospital follow-up
  • Medication & pharmacy issues
  • Chronic-disease management
  • Preventive reminders

Remote patient monitoring, where applicable

  • Blood pressure
  • Glucose
  • Weight
  • Pulse oximetry

Program participation, including remote monitoring, depends on eligibility, clinical need and applicable program requirements.

Capability

Built for complex care.

Multiple chronic conditions, complex medication regimens and heavy coordination needs are exactly what our team is built for. Introduce healthy and complex members alike.

  • Diabetes
  • Hypertension
  • Cardiovascular disease
  • COPD
  • Chronic kidney disease
  • Multiple chronic conditions

Quality

We do not wait for patients to fall out of care.

We look for opportunities to reconnect patients with preventive and chronic care instead of waiting for the next problem.

We actively identify patients due for

  • Annual Wellness Visits
  • Preventive screenings
  • Chronic-condition follow-ups
  • Labs and medication monitoring
  1. 01

Patients who may be due for care are surfaced.

  2. 02

Staff call directly, in English or Spanish.

  3. 03

The visit is booked on that same call.

  4. 04

    Follow up

Missed appointments are worked until the patient reconnects.

Our infrastructure is designed to support applicable quality and care-gap initiatives.

Transitions of care

After the hospital, the patient should not be alone.

After an ER visit or hospitalization, we reconnect the member with primary care and appropriate follow-up.

  • 01Follow-up appointments
  • 02Medication review
  • 03Specialist referrals
  • 04Ongoing coordination

Whole-family primary care

One medical home for the whole family.

We provide primary care across age groups, so the relationship does not stop with one Medicare member.

Children. Adults. Seniors. One trusted primary-care team.

  • Child
  • Parent
  • Grandparent

On-site services

More care.
Fewer stops.

  • Diagnostic Ultrasound

Imaging performed on site with the patient's own care team.

  • In-House Blood Draws

Routine labs completed where the patient is already seen.

  • Medication Support

Help arranging medications and, when available, home delivery.

Chavez & Mederos does not own or operate a pharmacy. Medication delivery depends on the pharmacy services available to the patient.

Technology

Personal care. Scalable infrastructure.

Our internal technology is being developed to help our team manage care management, outreach, follow-up, care gaps and operational workflows as the practice grows.

A patient-facing experience is on the roadmap — conceptual today.
